Information for Canadians, age 54 and under:



Canadians aged 54 and under: Simply fill out the application form and mail or fax it to us. You will be covered for any pre-existing conditions that have been stable for 180 days (90 days for high blood pressure, 365 days for heart conditions). You may also call us for more information or to apply by phone.

Information for Canadians, age 55 and over:



Canadians, age 55 and over: Begin by filling out the Medical Questionnaire. It will determine which rate category you fall under. If you answer Yes to any question in Part A or B, please contact us to determine the next steps you can take. You may need to use the following form:  Medical Underwriting
